An 80-year-old man with stage IV gastric adenocarcinoma presents with rapidly enlarging nodules of the lower lip and chin over five weeks. Histology revealed atypical gland-forming epithelial cells with hyperchromatic nuclei and frequent mitoses. Immunostaining was diffusely positive for pan-cytokeratin (AE1/AE3), weakly positive for CK7, and negative for CK20 and CD68. Ki-67 index was markedly elevated. What’s your diagnosis?
